Arizona state office has had birth
and death records since July of 1909 and also has abstracts of records
filed in counties prior to that date. Please make your check or
money order made payable to The Office of Vital Records.
Applicants must submit a copy of their picture identification or
have their request notarized to be processed. You should call the
recorded message at (602) 364-1300 first to verify the proceedure
for obtaining the documents.
